Living Arrangement Program

LIVING ARANGEMENT PROGRAM

TRADITIONAL CARE MODEL

Personalized Care in a Home Environment

Host family living arrangements, traditional care models are 24-hour residential alternatives to typical shift staff arrangements.  The setting is the residence for the person with a developmental disability and the full-time residence for the paid caregivers.  It is owned, leased or rented by the paid caregivers.  In traditional care settings, host families consist of one or more persons who are unrelated to the individual with a developmental disability and who are under contract with the provider agency to provide host family services.

​SHARED LIVING MODEL 

Integrated Support in a Home Setting

Our Shared Living Model offers 24-hour residential alternatives to traditional shift-staff arrangements. In this setting, individuals with developmental disabilities reside in a home that may include full or part-time caregivers, ensuring that over 50% of support is provided by dedicated individuals rather than shift employees. This approach fosters a stable, family-like environment, promoting independence and community integration.

PHILOSOPHY OF SERVICE

A Holistic Approach to Patient-Centered Wellness

Our philosophy of services is very simple – we strive to provide “patient-centered” care for our clients. We believe that to achieve optimal health and mental well-being, care needs to be holistic in nature since humans are bio-psycho-social-spiritual being; therefore, traditional medical-disease model of care is not enough.

In short, “Patient-Centeredness” means tailoring care to fit individual client’s health and mental well-being. This involves-

  • Refocusing care to the choices, goals for care, and preferences of the “patient’ rather than on efficiencies of the provider of services or supports.

  • Emphasizing and recognizing the individual patient’s self-determination, choices, worth, and unique set of values, views, histories, and interests.

  • Promoting a life-affirming, satisfying, humane, and meaningful experience.

The philosophy is guided by the following four (4) core principles:


i. Treating people with dignity, compassion, and respect.

ii. Providing coordinated care, support, and treatment.

ii. Offering personalized care, support, and treatment.

iv. Enabling patients to recognize and develop their strengths and abilities, so they can live an independent and fulfilling life.

COMMUNITY CONNECTOR

Building Confidence, Independence, and Opportunity

Anova Healthcare Services offers Community Connector (CC) services designed to empower individuals to thrive within their communities. Through these services, individuals are supported in developing meaningful relationships, building natural support networks, and achieving personal goals while promoting safety, independence, and confidence.

CC services utilize everyday community settings as immersive learning environments, providing individuals with real-world experiences to access their communities effectively and safely. These services expose participants to diverse cultural, social, recreational, volunteer, and employment opportunities, helping them explore and engage in the world around them.

Our approach emphasizes informed and confident community participation, enabling individuals to plan, make choices, and grow while accessing the resources and opportunities available to them.

HOMEMAKER SERVICES

Supporting Independence, One Task at a Time

Anova Healthcare Services provides Homemaker services to support individuals with intellectual and developmental disabilities in managing essential household tasks, enabling them to live as independently as possible in the community of their choice.


Delivered in the person’s home, these services aim to maintain a healthy, safe, and clean environment. Depending on the individual’s needs, Homemaker services may involve completing tasks on their behalf or providing guidance and prompts to encourage independence. The specific level of support and tasks are tailored to each individual, based on their personalized plan and coordinated with their Case Manager or Resource Coordinator. Services may include:

  • Laundry

  • Cleaning/Changing Linens

  • Folding/Ironing/Organizing Clothes

  • Dusting Furniture/Shelves/Surfaces

  • Vacuuming Carpets/Throws/Rugs

  •  Washing/Mopping/Cleaning Floors

  • General Bathroom Cleaning

  • General Kitchen Cleaning

  •  Cleaning Windows (Indoors)

  • Trash Removal

  • Sorting and Organizing

  • General maintenance

HOMEMAKER

NON-MEDICAL TRANSPORTATION

Reliable Travel for Meaningful Lives

Anova Healthcare Services offers Non-Medical Transportation designed to empower individuals with intellectual and developmental disabilities to access meaningful day activities. These services ensure safe and reliable travel between home, day programs, or employment settings, fostering independence and active community engagement.


By providing dependable transportation, Anova helps individuals stay connected, engaged, and involved in activities that enhance their quality of life. Whether traveling to work, a day program, or other vital destinations, our Non-Medical Transportation prioritizes safety, reliability, and accessibility.
