INTERDISCIPLINARY
POLICY

Ensuring Personalized, Inclusive, and Integrated Support

Our interdisciplinary policy is modeled based on Section 115.230 of the Illinois Department of Human Services guideline.

The needs of all individuals shall be addressed through an interdisciplinary committee which consists of the community support team namely the client or guardian; the client’s family member with no disability; medical professionals such as QMRP and QMHP; and other members of CST.

The interdisciplinary process shall involve:

 

  • Documentation of services being provided to the client prior to the development of a comprehensive integrated service plan by the QMRP or QMHP.

  • Development of an integrated services plan within 30-days of acceptance into CILA facility.

  • Initial assessment and re-assessment of client. Assessment results shall be discussed with the client and guardian as well as documented in the individual’s record.

  • Assessment shall include determination of individual’s strength and needs; racial, ethnic,and cultural background; chronological and developmental age; visual and auditory impairments; language preferences, and degree of disability.

  • Recurring annual re-assessment.

  • The integrated service plan shall identify CILA site and indicate the type and amount of supervision to be provided to the individual.

  • The plan shall ensure treatment activities addresses individuals’ daily independence, economic self-sufficiency, and community integration.

  • Plan shall include names and titles of all employees and signed by the individual or guardian as well as QMRP and/or QMHP and become part of the individual’s record.  

  • Plan shall be reviewed
