Host Family Living Arrangement Program

Traditional Care Model 

              Host family living arrangements, traditional care models are 24-hour residential alternatives to typical shift staff arrangements.  The setting is the residence for the person with a developmental disability and the full-time residence for the paid caregivers.  It is owned, leased or rented by the paid caregivers.  In traditional care settings, host families consist of one or more persons who are unrelated to the individual with a developmental disability and who are under contract with the provider agency to provide host family services.

Shared Living Model 

              Host family living arrangements, shared living models are also 24-hour residential alternatives to typical shift staff arrangements.  The setting is the residence for the person with a developmental disability and may house either full or part-time caregivers in which more than 50 percent of the residential coverage is provided by individuals other than shift employees. 

PHILOSOPHY OF SERVICE

Our philosophy of services is very simple – we strive to provide “patient-centered” care for our clients. We believe that to achieve optimal health and mental well-being, care needs to be holistic in nature since humans are bio-psycho-social-spiritual being; therefore, traditional medical-disease model of care is not enough.

In short, “Patient-Centeredness” means tailoring care to fit individual client’s health and mental well-being. This involves-

  • Refocusing care to the choices, goals for care, and preferences of the “patient’ rather than on efficiencies of the provider of services or supports.

  • Emphasizing and recognizing the individual patient’s self-determination, choices, worth, and unique set of values, views, histories, and interests.

  • Promoting a life-affirming, satisfying, humane, and meaningful experience.

The philosophy is guided by the following four (4) core principles:
 

i. Treating people with dignity, compassion, and respect.

ii. Providing coordinated care, support, and treatment.

ii. Offering personalized care, support, and treatment.

iv. Enabling patients to recognize and develop their strengths and abilities, so they can live an independent and fulfilling life.

OUR SERVICES

Community Connector (CC)

Anova Healthcare Services will provide CC services to individuals in the community. CC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals. Anova Healthcare Services will utilize typical community settings as part of the CC learning environment.

CC services will provide individuals with experience accessing their community competently, confidently and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and future employment opportunities available to them in their community; and, educate individuals about how to access their community in an informed manner. CC will provide individuals the opportunity to plan, choose, and learn while accessing their

Homemaker

Anova Healthcare Services will provide Homemaker services designed to support persons with intellectual and developmental disabilities who require assistance to manage basic household tasks necessary to live as independently as possible in the community of their choice.

Homemaker services will be provided in the person’s home and allow the person to maintain a healthy, safe and clean home environment. Homemaker services may involve completing the home task for the person or providing cueing to prompt the person to complete a home task. The level of support and tasks to be completed will be identified in the person’s Individual Plan and approved by the person’s Case Manager/Resource Coordinator. The following household support may be provided as part of Homemaker services:

 

  •  Laundry

  • Cleaning/Changing Linens

  • Folding/Ironing/Organizing Clothes

  • Dusting Furniture/Shelves/Surfaces

  • Vacuuming Carpets/Throws/Rugs

  •  Washing/Mopping/Cleaning Floors

  • General Bathroom Cleaning

  • General Kitchen Cleaning

  •  Cleaning Windows (Indoors)

  • Trash Removal

  • Sorting and Organizing

  • General maintenance

Non-Medical Transportation

Anova Healthcare Services will provide Non-Medical Transportation services to individuals living in the community. Non-Medical Transportation is designed to provide individuals with intellectual and developmental disabilities the opportunity to access meaningful day activities. Non-Medical Transportation will support the person to safely travel to and from their home and their day program setting or job.

Non-Medical Transportation services will allow the individual to remain active and engaged in meaningful day activities and provide the person a safe and reliable transportation service.
Anova Healthcare Services will provide Non-Medical Transportation services to individuals to and from their home and day program services.
